He served as a professor of Psychiatry at the University of Ghana and held the presidency of the West African College of Physicians from 1983 to 1984. Forster was actively involved in various medical organizations, being a foundation member of the Medical Research Council of Psychiatry, a fellow at the Royal Society of Arts, and a member of multiple psychiatric associations across West Africa and the USA.
