Jan Dawidziuk was a Polish-American bishop of the Polish National Catholic Church, serving as the sixth diocesan bishop of the western diocese from 2003 to 2009. Born on December 17, 1937, in Kolczyn, Poland, he was consecrated in November 1999. His diocese included parishes across several states in the U.S. He worked from the Chancery located in Park Ridge, Illinois, and was based at All Saints Cathedral in Chicago. He passed away on March 6, 2012, in Parma, Ohio.
