Relf faced several health issues throughout his life, including emphysema and asthma. He tragically died at the age of 33 on May 12, 1976, after suffering an electric shock while playing a poorly grounded guitar in the basement of his home. Although many sources incorrectly state May 14 as his date of death, his official death certificate confirms May 12.
